Summary Care Record

Your Summary Care Record contains important information about any medicines you are taking, any allergies you suffer from and any bad reactions to medicines that you have previously experienced. Allowing authorised healthcare staff to have access to this information will improve decision making by doctors and other healthcare professionals and has prevented mistakes being made when patients are being cared for in an emergency or when their GP practice is closed.

You may wish to add more information about your care to your Summary Care Record – this is called ‘Additional Information’ and includes the following: significant medical history (past and present), reason for medication, anticipatory care information (such as information about the management of long term conditions), end of life care information & immunisations. If you would like more information to be added to your SCR to support your care, please let your GP practice know. What are my choices?

If you choose to have a Summary Care Record, you do not need to do anything as a Summary Care Record is automatically created for you.

If you would like to have Additional Information added to your SCR, please download and complete the following form and return it to your GP practicehttps://digital.nhs.uk/media/1181/SCR-and-additional-information-patient-leaflet-and-consent-form/pdf/scr_additional_information_leaflet_consent_form1, or alternatively simply let your GP practice know and they will update your consent status.

If you do not wish to have a Summary Care Record at all, you can opt out by filling in and returning an opt-out form https://digital.nhs.uk/summary-care-records/patients. Opt-out forms can be downloaded from the website or you will be able to complete a form from reception.
